Vaccinations update - November 2021 - influenza and COVID-19

The NHS childhood immunisation team will be visiting school during November to administer influenza and COVID-19 vaccinations to students with parental consent.
Please see both letters below which detail the two separate consent processes for these two vaccinations. Please read these letters and complete the online consent form(s) if you would like your child to receive one or both of these vaccinations.
Both consent forms require the school code for Outwood Academy Bishopsgarth: 143146

Influenza vaccination - offered to all students.
COVID-19 vaccination - offered to all students age 12 and over.
